He is survived by: his wife Margaret Smith; his daughters, Jennifer Church and Lori Slye (Richard); his siblings, Cheryl Moore (Danny) and Jeff Smith (Laura); and his grandchildren, Audrey Gregg (Hunter), Grant Slye, Abby Slye, Luke Slye and Ava Church. He is also survived by several nieces, nephews, extended family members, and dear friends.
