He is survived by: his wife Amy Self; his daughters, Deanna Nicole Self of Cleveland and Hailey Grace Self of Cleveland; his father Michael D. Self; his sisters, Angela Denise Self Friedman (Mike) and Kayla Deanna Self; and his grandmother Lavida Sue Self (Cleveland). He is also survived by several nieces and nephews.
