He is survived by: his sons, Josh Ptak (Donna), Jeremy Ptak and Daniel Shangle (Heather); his siblings, Julie Lawrence, Tom Ptak (Kathy) and John Ptak; and his grandchildren, Emilee Ptak, Madelyn Ptak, Brylee Ptak, Raelyn Ptak, Annalise Shangle, Renek Howell and Demarcus Howell Jr..
Visitation will be held on Tuesday, October 13th 2026 from 12:00 PM to 4:00 PM at the Companion Funeral & Cremation Service - Cleveland (2415 Georgetown Rd NW, Cleveland, TN 37311).
In lieu of flowers, donations may be made to the family.
