She was predeceased by : her parents, Johnny Phy and Sarah Phy (Norris); and her children, Lori Ann Neal and Jeffrey Scott Stewart. She is survived by : her husband Hugh Carmack of Cookeville; and her daughters, Brenda Wells of Cookeville, Sherrie Ann Stewart of Cookeville, Phyllis Tower (David) of Baxter and Teresa Wade of Cookeville. She is also survived by Many brothers, sisters, grandchildren, and great grandchildren.

Visitation was held on Monday, May 13th 2024 from 11:00 AM to 8:00 PM and on Tuesday, May 14th 2024 at 10:00 AM. A funeral service was held on Tuesday, May 14th 2024 at 2:00 PM at the Whitson Funeral Home - Cookeville Chapel (30 N Dixie Ave, Cookeville, TN 38501).
