He was predeceased by: his parents, Charlie B. Cook and Helen Cook; his son Brent Lee Cook; and his siblings, Patricia Stoots and Mike Cook.
He is survived by: his children, Wade Cook Martin (Donna) of Brighton, Chandra Cook of Erin, TN and Caley Baker of Erin, TN; and his siblings, Cathy Chandler (Buddy) of Covington, Janice Ferrell of Springville, TN, Cheryl Goode of Trezevant, TN and Steve Cook of Covington. He is also survived by eight grandchildren.
