He was predeceased by: his parents, Jesse W. Hawk and Mary Nell Blevins Hawk; his daughter Jacqueline Caudle; and his granddaughter Chelsea Marie. He is survived by: his wife Evelyn Hartley Hawk; his children, Jonathan C. Hawk and Jennifer Phelan (Charles Phelan); his grandchildren, Georgia Caudle, Shelayne Phelan and Clayton Phelan; and his brothers, James Hawk (Darlene) and David Hawk (Karna). He is also survived by several nieces and nephews.
