He was predeceased by: his father Herb; and his grandchildren, Don-Michael Slack and Abra Perian. He is survived by: his sister Anna Reba; his brother-in-law Carl Conner; his wife Wanda Moore Slack; his children, David (Jan Slack), Michael (Teri Slack), Deborah (Tim Smith) and Charles (Rita Slack); and his grandsons, Derek Slack, Heath Slack, Marcus Adams, Travis Smith, Doug Benton, Nick Carson, Zach Burnette, Weston Smith, Malcolm Measles and Jacob Hicks. He is also survived by numerous grandchildren and great-grandchildren; several nieces and nephews, as well as friends.
