He was predeceased by: his parents, King David and Lizzie Jane Mayfield; his siblings, Walter Raymond Mayfield of James Daniel, David Carroll Mayfield, Roy Quay Mayfield, Dorothy Ruth Nichols and Margaret Annaline Lewis; his former wives, Blanche T. Mayfield and Leslie Faye Mayfield; and his son Douglas William Mayfield.
He is survived by: his sisters, Opal Joyce Belcher and Betty Lou (Jim Miller); his granddaughter Gretchen (Duane Ferguson); and his great grandchildren, Makenzie and Makade Kittle. He is also survived by 16 nieces and nephews; step-son.
