He was predeceased by: his parents, John Phillip Small, Sr. and Julia Isabel Fox Small; and his brother James Small. He is survived by: his wife Vickie Allen Small; his children, John Anthony Sack (Tonya), Sondra Luker (Robert), Christine Bankston (Boo) and Phillip Small; and his grandchildren, Julie Bankston, Carter Bankston, Isabella Luker, Colin Luker, Brittany Klatt, Madison Evans and Jacob Miller.
