He was predeceased by: his parents, James Marvin Cook and Goldalea Taylor Cook; and his siblings, Judy Davis (Calvin Davis), James Daniel Cook and Cass David Cook.
He is survived by: his wife Marianne Cook; his sons, Phillip "Brian" Cook (Haden Wiley Cook) and Clayton Taylor Cook (Kelly Postiglione Cook); and his grandchildren, Olivia Taylor Cook, Charles Fisher Cook, David Wiley Cook, Brooklyn Grace Weatherford and Clayton Patrick Cook.
