He is survived by: his wife Carolyn Forkum; and his sisters, Connie Shepherd (Wade) of Tampa and Brenda Messersmith (Ray) of Brooksville, Florida. He is also survived by his 3 sons; his 3 step daughters; his 1 stepson; his numerous grandchildren.
