He is survived by: his wife Susan Forsythe of Humboldt, Tennessee; his children, Cody Forsythe (Jessica) of Medina, Tennessee and Mandy Olson (Eli) of Gibson, Tennessee; his siblings, Jimmy Forsythe of Beech Bluff, Jessie Forsythe of Gadsden, David Forsythe of Milan, Tennessee and Kelly Brown of Dayton, Tennessee; and his grandchildren, Alivia Olson, Garrett Olson, Coldyn Forsythe, Thomas Chenoweth, Dustin Forsythe, Kendal Forsythe, Kelsy Forsythe and Skyler Forsythe.
