He was predeceased by: his parents, Brooks Polk and Irene Dale Polk; and his sisters, Jacqueline Polk and Jerry Beth Delauney. He is survived by: his wife Marilyn Polk of Jackson, TN; his children, Douglas Dale Polk, Jr. (Christie) of Jackson, TN, Dean Polk (Lee) of Harvest, AL and Daisy Polk; his siblings, Brooks Polk, Jr. (Betty), Donna Griffith (Jim) and Kay Garrison; and his grandchildren, Gabriel, Brooks, Parker, Jude, Presley and Ben.

Visitation was held on Friday, April 18th 2025 from 10:00 AM to 12:00 PM at the Englewood Baptist Church (2239 N Highland Ave, Jackson, TN 38305). A funeral service was held on Friday, April 18th 2025 at 12:00 PM at the same location.
