He was predeceased by: his daughter Kim Hicks; his parents, Melvin Slaven and Nora Slaven (Anderson); and his brother Tim Slaven Troy Slaven. He is survived by: his children, Karen Hurst (David Wayne) and Jesse Hicks (Brittany); and his siblings, Mark Slaven Cody Slaven and Pam Johnson (Chuck Sam Atkins). He is also survived by 6 Grandchildren, nieces, nephews, other family and friends.
In lieu of flowers, the family has requested donations to be made towards funeral expenses.
