He was predeceased by: his father Cordell Cook; and his pet Blue. He is survived by: his mother Charlene Cook (Peggy Haley); his siblings, Jimmy Cook (Pat) and Sandy Thompson (Tim Sherry Vincent); and his siblings' children, Cameron, Christin, Daniel, Bradley, Amanda and Sarah. He is also survived by 9 Great Nieces and Great Nephews.
In lieu of flowers, the family has requested donations to be made to the Westfelt Cemetery Fund.
