He is survived by: his stepfather Patrick Costello III of Johnson City, TN; his father David Holtz; his stepmother Donna Holtz; his half-sister Shana Newland (Sam) of Washington, Indiana; his stepsiblings, Karen Costello of Phoenix, AZ, Patrick Costello IV of Johnson City, TN, Nicholas Costello (Chelsea) of Fultonville, NY and Krystal Clark (Justin) of Johnson City, TN; his mother Deborah Costello; Tonya Parker (Bart) of Morristown, TN.. He is also survived by numerous cousins, nieces and nephews.
