He is survived by: his wife Lisa Bivins Cook; his stepdaughters, Misty Donald Beard and Monica Donald Blythe; his sisters, Kathi Cook Taylor (Tom) and Joni McAnnaly; his nephews, Chris Richards, Shane Crane (Renee), David Crane (Chasity) and Randall Ventress; and his niece Kaila Taylor. He is also survived by seven grandchildren, 4 great grandchildren.
