He was predeceased by: his parents, Dan Jacques and Marian Steffa Jacques. He is survived by: his wife Tracy Sullivan Jacques; and his sons, Kevin Jacques of Jacksonville, FL, Aaron Greene of Birmingham, AL and Talan Greene of Jacksonville, AL. He is also survived by five grandchildren and one great grandchild.

In lieu of flowers, the family requests donations be made in his memory to St. Jude Children's Research Hospital or to the Wounded Warriors Fund.
