He was predeceased by: his parents, Otis Cook and Louis Cook (Mathis). He is survived by: his wife Karen Cook; his son Carl Cook (Wanda) of Savannah; and his stepchildren, Shane Spears of New Johnsonville, Kara Flippin (Chris) of Huntingdon, Heather Dieckmeyer (Brian) of Henderson and Tessa Page (Brian) of McKenzie. He is also survived by 10 step grandchildren and 10 great grandchildren.
