He was predeceased by: his parents, Charles A. Smith and Inez Smith; and his brother Edward Lee Smith. He is survived by: his wife Anita Smith; his children, Dr. C. Michael Smith, Jr. (Gina), Karen Dickinson (David) and Allison Swearingen (Greg); and his brother Dr. Vincent Smith.
