He is survived by: his wife Audrey Slomkowski; his children, Heather Slomkowski-Markle (James Beasley), Amber Baughman (Matthew), Megan Cowan (Wayne), Steven Adam Slomkowski and Hudson Slomkowski; his siblings, Mary Piatak (Jan), Jane Griffith (Frank), Susan Buzzard (Bruce) and Frank Slomkowski (Peggy); and his pet Jack. He is also survived by eleven grandchildren; one great grandchild.
In lieu of flowers, the family asks that any memorial contributions be made to the American Heart Association or the American Cancer Society in his name.
