He is survived by: his wife Regina; his children, Corey Cook of Washington, Dustin Holocker of Illinois and Brenna Zittle of Louisiana; his brother Duane Cook (Angie) of Louisiana; and his siblings-in-law, Randel and Brisha Collins of Washburn, TN. He is also survived by 6 grandchildren and 1 great-grandchild; and many nieces and nephews.
