He was predeceased by: his parents, Helen Shockley and Howard Shockley; and his brother Mark Shockley. He is survived by: his wife Sandy K. Shockley; his sons, Joshua Shockley (Rhonda) and Adam Shockley (Andrea); his grandchildren, Keaton, Loryn, Easton and Harper Shockley; and his siblings, Connie Beck, Bill Shockley (Ginger) and Tim Shockley (Sandra). He is also survived by several nieces and nephews.

In lieu of flowers, memorial donations in David's name can be made to the ALS Association.
