He was predeceased by: his wife Billie Jo Hazelwood; his parents, Ervin Hazelwood and Lena Hazelwood; his parents-in-law, James and Dussie Ailshie; his sister Helen; his brother-in-law Alfred McKinney; and his sister Deanie Barbee.

He is survived by: his children, Carolyn McCarter (J. B.) and Rev. Chris Hazelwood (Christie); his grandchildren, David Slagle (Haley) and Desirea Rader (Richie); his great grandchildren, Chloe Slagle, David Slagle, Forrest Slagle, Cade Rader and Zane Rader; his caregivers, Aggie Wood and Yelonda Johnston; Jo Long. He is also survived by several nieces and nephews.
