He was predeceased by: his parents, Lon Cook and Rachel Ammons. He is survived by: his wife Thelma Cook; and his children, Lisa Cook, Teresa Smith and Steven Reavis. He is also survived by many grandchildren, siblings, nieces, and nephews.
