He was predeceased by: his wife Dora Lois Jack (Bowser) of Indiana; his parents, Lewis Jack and Wilda Jack (Claypool); and his siblings, Norman, Gerald, Marlan, Dwight, Lynol, Kay Bentley and Eunice Streich.
He is survived by: his children, Priscilla Anderson (Jim), Pauline Pickering (Ray), Tim Jack (Kim), Phyllis Kenderdine (Lou), Twila Forster, Phil Jack (Leah) and Pam Van Der (Peter); and his brother Corbin Jack of Shelocta, Pennsylvania..
