He was predeceased by: his former wife Nancy Levi; and his brothers, L. W. and Sidney Levi. He is survived by: his wife Janice Levi; his children, Jennifer Levi, Jon Levi (Kristi), Kenny Meredith (Jessica), Travis Millsaps (Gail) and Scott Millsaps; his grandchildren, AnnaLevi Chavis (Jay Garland), Walker Chavis (Cassie Byrd), Zoe Chavis, Canaan Chavis, KaraLynne Bargatze (Dylan), Madison Alig (Bryce), Roman Meredith, Victoria Meredith and Bridget Millsaps; and his great grandchildren, Leo Chavis, Blaire Bargatze, Emsley and Luna Alig.

Visitation will be held on Monday, September 21st 2026 from 3:00 PM to 5:00 PM at the Woodfin Memorial Chapel (1488 Lascassas Pike, Murfreesboro, TN 37130). A memorial service will be held on Monday, September 21st 2026 at 5:00 PM at the same location.
