He was predeceased by: his great-great-grandparent Lena "Meems" Bourgeois. He is survived by: his parents, Blaise and Brianna of Nashville, TN; his sisters, Eloise, Agnes, Amos-May and Greta Ruth; his grandparents, Daniel, Darcy MacEachern of Augusta, GA, Kevin, Jessica Poe of Nashville, TN and Tad Dillon of Savannah, GA; his great-grandparents, Richard, Linda Bourgeois of Collins, GA, Phyllis MacEachern of Manchester and Harriet Dillon of Savannah, GA; and his aunts, KC Prather (Stuart), Kathleen Moeller (Nick) and Autumn Watson. He is also survived by numerous great aunts, great uncles and extended cousins.
In lieu of flowers, the family kindly asks that donations be made in honor of Jan-Blaise to the following three organizations that served Jan-Blaise, and never once charged for their services.
