He was predeceased by: his parents, Elmer Cook and Elsie Cook; his wife Betty Cook; and his siblings, Mona Wells, George Cook and Betty Lou Cook.

He is survived by: his children, Vicki Cook and Benjamin Cook (Veronica); and his brother Edward Cook (Karan). He is also survived by 9 grandchildren, 2 great-grandchildren, and many nieces and nephews.
