He was predeceased by: his father Wayne Wormsley. He is survived by: his wife Sharon Cates Wormsley; his sons, Matthew, Mitch and Mark; his grandchildren, Isaac, Blake, Kara and Cheyenne; his great grandchildren, Braden, Adalyn, Winter, Summer and Jayce; his mother Janice Adcock Wormsley; his sisters, Freda Davidson and Annette Surber; and his close friend Tom Grizzard. He is also survived by many nieces, nephews, cousins and other family members and friends.
