He was predeceased by: his parents, Guy Wilson Cook and Clara Dorothy Gillihan-Cook; his wife Virginia Nell Cook (Stout); his sisters, Joyce Bush (Cook) and Jonelle Carter (Cook); his son-in-law Tony Keith; and his great grandchild Jessica Keith.
He is survived by: his children, Doug Cook (Bonita) of Portland, Nancy Smith (Thomas) of Portland, Ellen Keith of Portland and Julie Patterson (John) of Portland; his grandchildren, Derek Vandercook, Dana Abreu (Victor), Rusty Keith (Heather) and Natalie Robertson (Gavin); and his siblings, Ralph Cook (Diana) and Brenda Knight (Jimmy). He is also survived by seven great grandchildren; five great great grandchildren.
