He was predeceased by: his parents, Holly Rufus Forkner, Sr. and Elizabeth Forkner (Poer); and his grandsons, Nathan Forkner and Seth Freeman. He is survived by: his wife Betty Jo Forkner (Motley); his children, Gayle (Steve) and Charles Scott Forkner (Joanie) of Versailles, KY; and his stepsons, William Stewart Gash II (Suzanne) and David Bruce Gash (Tina) of Smithville, TN. He is also survived by 6 grandchildren; 4 step-grandchildren; 9 great-grandchildren, and 7 step-great-grandchildren.
In lieu of flowers, the family has requested that donations be made to Live and Let Live.
