He is survived by: his caregiver Janice Hood Apple; his daughter Dorothy Melissa Creekmore (Michael James); his grandchildren, Michael James Creekmore II, Cathryn Grace Creekmore and Christopher Lee Cook (Jeanna Marie); his great grandchild Evelyn Grace Cook; and his granddaughter Melanie Ann Savoca.
