He was predeceased by: his parents, Dr. Frederick Cieslak and Dorothy Cieslak. He is survived by: his wife Mary Susan Ballard; his son Christopher (Julia); his grandchildren, Heidi, Benjamin, Cecilia, Jenna (Jesse), Aiden, Ashden and Alden Womacks; his brothers, Michael Cieslak (Carmelle), Martin Cieslak (Pat) and Matthew Cieslak (Bernadette); his brother-in-law Timothy Ballard; his daughters, Lucy Lee (Jason) and Chloee Choi; and his mother-in-law Sue Huston. He is also survived by many nieces and nephews.

In lieu of flowers, memorial contributions may be sent to Faith Baptist Church, P.O. Box 132, Harriman, TN 37748, designated for Mission Support.
