With solemn hearts, we announce the passing of Mitchell Frederick Cieslak of Spring City, Tennessee, whose presence will be deeply missed, having departed on September 30, 2025 at the age of 62. He was predeceased by: his parents, Dr. Frederick Cieslak and Dorothy Cieslak. He is survived by: his wife Mary Susan Ballard; his son Christopher (Julia); his grandchildren, Heidi, Benjamin, Cecilia, Jenna (Jesse), Aiden, Ashden and Alden Womacks; his brothers, Michael Cieslak (Carmelle), Martin Cieslak (Pat) and Matthew Cieslak (Bernadette); his brother-in-law Timothy Ballard; his daughters, Lucy Lee (Jason) and Chloee Choi; and his mother-in-law Sue Huston. He is also survived by many nieces and nephews.
Visitation will be held on Monday, October 6th 2025 from 10:00 AM to 12:00 PM at the Baker-Stevens-Parramore Funeral Home (6850 Roosevelt Ave, Middletown, OH 45005). A funeral service will be held on Monday, October 6th 2025 from 12:00 PM to 1:30 PM at the same location. A burial will be held on Monday, October 6th 2025 at 2:00 PM at the same location.
