He is survived by: his brother Timmy Hazelwood (Lisa Clark) of Lewisburg, TN; his stepmother Sonja Hazelwood of White House, TN; his stepsiblings, Savannah Johnson of Lafayette, TN and Josh Smith (Patricia) of Westmoreland, TN; his grandmother Dora "Dodie" Hazelwood; his aunt Sharlene Hazelwood; his uncle Doug Hazelwood; and his cousins, Casey Paris of Berlin Community, Dwayne Sullivan of Cornersville, TN, Katlyn Sullivan of Columbia, TN and Caitlin Sullivan of Columbia, TN. He is also survived by several nieces, nephews, and cousins.
