She was predeceased by: her mother Janice Hester McCormack; her grandparents, Jeff, Eula Hester, John Cartwright "JC" and Olean McCormack; and her nephew Bobby McCormack.

She is survived by: her father Bobby McCormack; her sons, Matthew McCormack and Luke Trent; her sisters, Sherry Myers (Ken), Sharon Costa and Shannon Baron (Tommy); her grandchildren, Magnolia and Winston McCormack; her nephew Tyler Myers (Alicia); her niece Amy Newton (Brandon); and her great-nieces and great-nephews, Emerie, Emilia Newton, Harrison and Josie Myers.

In lieu of flowers, memorial contributions may be made to Alive Hospice or Sarah Cannon Cancer Center in honor of Brooke McCormack.
