He is survived by: his wife Barbara Holt; his daughter Christina Thornton (Tim) of Amarillo, Texas; his siblings, Dr. Scott Middleton (Pam) of Craig, Colorado and Sharon Mathis (Nick) of Craig, Colorado; his granddaughter Maelie Sullivan of Amarillo, Texas.; and his nieces and nephews, Cheryl Mize (Glen), Jordan Anderson (Mark), Jillian Schneider (Jason), Catlin Donato (Jason), Amy Mathis, Allison Mathis, Ben Carroll (Iona), Brad Carroll (Danna), Michael Mathis (Amanda) and Stephen Mathis.
