He was predeceased by: his parents, Al Joseph Cook and Snow Flake Ballinger Cook; and his brothers, James Ellsworth and Charles Edwin. He is survived by: his wife Mary Jo Cook; his daughters, Shawn Curtis (Mike), Cindy Hickman (Chris) and Amy Homeyer (Ken); his grandchildren, Patrick Curtis (Ashley), Katie Curtis, Sydnee Foster (Tyler), Molly Giles (Griffin), Madeleine Hickman and Charlie Hickman; and his great grandchildren, Finley, Audrey, Grayson and Adelaide.
In lieu of flowers, contributions can be made to the Grace Lutheran Church Pavilion Fund, honoring Sid's memory and his commitment to his church community.
