He is survived by: his brother Kenneth Polk (Sue Nordhausen Polk); his children, Terry Polk (Jeanne Steckler Polk), Denise Polk Thornton (Chap) and Randy Anthony (Michelle); his grandchildren, Dillon Polk (Emily Hill Polk), Brandon Polk (Mary Lockeby Polk), Katelynn Thornton, Jenna Thornton and Ryder Thornton; and his great grandchildren, Wyatt Polk, Everitt Polk, Hazel Polk, Valerie Polk and Garrett Polk.
