She was predeceased by: her parents, Wardell Howard Creamer and Helen Cusick Creamer; her siblings, Howard Creamer and Wanda Creamer; her parents-in-law, Charles Horak, Sr. and Thelma Horak; and her sister-in-law Tracie Kelley.

She is survived by: her husband Charles Horak, Jr.; her sister-in-law Linda Creamer; her children, Charles Horak III (Ann), David Horak (Anne) and Sharon Mathes (Tim); her grandchildren, Connor (Christian), Katherine, Austin, Richard, Jacob (Lindsey), Parker, Rachel, Hannah and Joseph; her great grandchildren, Olivia, Avery and Emma; her nephew Will Kelley; and her niece Terri Lutrick.
