He was predeceased by: his parents, Charles Otto Maxwell and Opal Maxwell; his wife Mary Ellen Maxwell; his daughter LeeAnn Clark; his grandsons, Brandon James Maxwell and Jeremiah Gage Byrd Sr.; and his siblings, Teddy, Gene, Ralph, Donnie, Lee, Pauline and Maudie.

He is survived by: his children, Theresa Kay Eckert (Ronnie), Forest James Maxwell Jr. (Melissa) and Danny Allen Maxwell (Melinda); his grandchildren, Kayley Gwinn (Justin), Chad Eckert (Lauren), Melinda Hernandez (Brian), Aspen Maxwell, Shayla Maxwell (Hannah), Kallen Maxwell and Darien Maxwell; and his sisters, Delores and Lillian. He is also survived by several great-grandchildren.
