He was predeceased by: his parents, John J. Dvorak and Emma Dvorak (Dirba); and his sisters, Leona Gerik (Eugene), Mary Young (Durwood) and Evelyn Kolar.
He is survived by: his wife Patricia Dvorak (Patsy) of Elm Mott; his children, Raymond Dvorak (Linda), John Dvorak Jr. (Amanda) and Michelle Miller (Donnie); his grandchildren, Peyton Dvorak, Garrison Dvorak, Matthew Dvorak, LauraLayne Dvorak, Thomas Dvorak and Savannah Miller; his sister Georgie Hutchison; and his brother-in-law Raymond Kolar. He is also survived by many nieces, nephews, other relatives and friends.
In lieu of flowers, memorials may be made to the West VFW #4819 Womens Auxiliary.
