He was predeceased by: his parents, Charles Dolezal and Ida Dolezal (Cinek); and his sisters, Dorothy Dolezal and Betty Ender. He is survived by: his wife Mildred Ann; his daughter Linda Ann Jurik (Mitch); his grandchildren, Michael Jurik (Sarah), Amanda Jurik (Steffen) and Chad Jurik; his great grandchildren, John Jurik, Forrest Jarrett and Micah Jane Jurik; his nephew Douglos Ender; and his nieces, Michelle Holder and Janice Hodgson.
