He was predeceased by: his grandfathers, J. William Moore, Sr. and Mark Mandala; and his uncle Michael Mandala. He is survived by: his parents, Amy Moore Mandala of Tucson and Charlie Mandala of Dallas; his brother Michael; his grandmothers, Sarabeth Moore of Paradise Valley, AZ and Joan Mandala of La Quinta, CA; his aunts and uncles, Steve (Sheryl Mandala), Carrie Mandala, John (DeeDee Moore) and Anne Moore (Vinny Allen); and his cousins, Nicole, Kelly, Max, Katie, Emma and John.
In lieu of flowers, donations requested for Tucson Community Tennis Program at tctp.org to share Jack's love of tennis with underprivileged children in Tucson.
