He is survived by: his parents, Sandra Kay Jones and John William Lee Polk; his daughters, Sandra Polk, Chasity Villagomez and Kimberly Polk; and his siblings, Jennifer Jones and Darrell Jones. He is also survived by nine grandkids; and numerous cousins, nieces and nephews.
