He is survived by: his wife Rosemary Johnson; his children, Sean Mayfield (Cindy), Angela Sloan, Monte Mayfield, Joshua Mayfield (Lisa) and Kady Donaghey (Eddie); and his siblings, Elaine Norris (Jimmy) and Jim Mayfield (Jan). He is also survived by 18 grandchildren; 9 great-grandchildren.
