He was predeceased by: his parents, Truman Cook and Dorothy Blevins Cook; and his sister Ruby Cook Parks. He is survived by: his wife Angela Hefner; and his sisters, Sharon Swenson and Wanda Helton. He is also survived by many nieces and nephews.
