He was predeceased by: his siblings, Kim Campbell, Jamee Kramer and Jeffery Forrest; and his parents, John Micheal Forrest, Sr. and Jeanne Taylor Forrest.

He is survived by: his significant other Cheryl Diane Forrest of Ogden Utah; his children, Shane Forrest (Angela) of Irving, Texas, Trent Forrest (Angela) of Millsap, Texas, Erik Forrest (Deanne) of Dickinson, Texas and Melanie Forrest (Scott Walsh); his grandchildren, Madison Kile, Abigail, Isabell, Natalie Forrest, Paxton, Savannah, Phoenix Walsh, Cecilia, Jeffery, Arial Forrest and Shelby Rasmussen; and his great grandchildren, Harper, Carter and Conner Forrest.
