He was predeceased by: his parents, James Blake and Betty Lou Blake; and his brothers, Mike, Steve and David.

He is survived by: his daughter Brittany; his grandchildren, Tatum and Tucker; his son Sam (Hannah); his granddaughter Diane; his brother Tom (Nadine); and his sister-in-law Anne. He is also survived by numerous nieces and nephews, and dear friends.
