He is survived by: his parents, Steve Lively and Bonnie Lively of Hideaway; his brothers, Phillip Lively (Breanna) of Caddo Mills and Benjamin Lively (Alaina) of Lemoore, California; and his nephew Peter Lively. He is also survived by other loving family members and friends.
In lieu of flowers the family has asked that contributions be made to the Sam Lively Memorial Fund at Southside Bank, Lindale, Texas.
